Immunization records change set for next month

DAILY NEWS STAFF REPORT

Summit County Public Health (SCPH) is changing computer systems for immunization records next month. With the change, the organization will begin the Colorado Immunization Information System also known as CIIS. CIIS is a registry or database for immunizations. All immunization records from SCPH will be sent to this state database. If you would like to remove your child’s records from this database, you must obtain an opt-out form from SCPH then send the form to CIIS. Call SCPH at (970) 668-5230 by Jan. 15, 2006, to obtain an opt-out form.
